Directions for Submitting a PDF Form

To submit a PDF form, please follow the directions that appear when clicking “submit”. 

The easiest way is to submit the data is to select “Internet Email” in the “Select Email Client” window.  Click OK.  Next, click “Save Data File” and save the data in an easy to find location on your computer.  Finally, attach the data file to an email and send it to the appropriate email address with the subject line reading the title of the application.

If you have trouble sending the form via email, please type your answers on the computer, print the form and fax to (407) 823-3569.
